eForms Examples
Examples of eForms are provided in PDF and HTML format. Each eForm is documented to show the features and benefits.
- Accessibility - the tabbing order, section hierarchy and tags have been added to ensure that this document is accessible to various assistive technologies
- Conditional Regions – by selecting the type of leave at the top of the form the user can decide which version of the form they intent to complete. This cuts down not only on the number of paper forms but also consolidates the number of forms within an organisation
- Adding Areas – when using paper forms it often happens that the user may run out of space to write and has to use additional paper – this form allows extra rows to be added
- Offline Use – PDF documents can be Reader Extended* so that that users with Adobe Reader can complete and save forms thus allowing form completion to be carried out at the user’s pace and local versions can be stored electronically rather than paper versions.*
- User Enhancement – this form provides a calendar feature which displays when a user clicks inside the date fields and can help to speed up completion of the form as well as ensuring the date format is inserted as required

- Expandable Fields – the Delegates field has been made expandable so that multiple names can be entered and will expand the field and move content down the form and onto additional pages if required
- Duplicate Data – where an eFrom requires data that the user has already entered the eForm should be able to add this without having the user retype. The Invoicing Details section has a check box for “Same as Company Details” which will auto-populate this section with the details already entered
- Locked Regions – the signature and date fields at the base of the form are not enabled unless the check box agreeing to the terms and conditions is checked
- User Enhancement – this form provides a calendar feature which displays when a user clicks inside the date fields and can help to speed up completion of the form as well as ensuring the date format is inserted as is required for this form
